Sorry, we identified a small error. Mailing Address must be alphanumeric
We have noticed an error in the 'Date of Birth' field, i.e. which indicates the applicant is over 18 years. PLEASE NOTE that this MINOR application is only meant for: (i) those age 18 years and below , and (ii) if you are the Parent/Guardian applying for the MINOR. If you are 'over 18 years' and you are not applying for the MINOR, please click the "OVER 18" button below, and apply for the BIG Card (Loyalty) for yourself. If you are the Parent/Guardian applying for the MINOR, please click the "BELOW 18" button below and insert the correct Date of Birth of the MINOR to continue with the application. Please kindly enter again. Thank you. |
